- Do you provide a Business Associate Agreement (BAA)?
- Yes. We issue a standard BAA on request before your first pickup. Custom language for larger practices is negotiable.
- What's your destruction security level?
- Cross-cut to DIN 66399 P-4 standard — particles small enough to prevent reconstruction with commercial software.
- Who has access to PHI between pickup and shred?
- Only trained, background-checked DocuShred staff. Your shipment is locked in transit and logged at every handoff. Full chain of custody log available on request.
- What records do you keep of our shredding?
- We retain chain-of-custody logs for 7 years, per HIPAA recommendation. Certificates of Destruction are issued on request and similarly archived.
- What if a breach happens?
- Our insurance covers commercial pickup and destruction. We cooperate fully with any breach investigation. Never happened — but we're ready if it does.
- Can you handle dental-specific records?
- Yes. We shred for dental offices with the same HIPAA workflow as medical — X-ray film excluded, paper only.
